1.  Complete Your New Patient Forms

 

Once you book your appointment, you’ll receive an email with your digital intake paperwork.
Please complete all forms at least 24 hours before your visit so I can review your history, symptoms, and goals in advance.

This helps us maximize your time and ensures your care is fully personalized from the moment you walk in.


*If the patient is 17 or younger, please have the legal guardian sign the health history questionnaire*

2. What to Bring and Wear
 

Bring:

  • A photo ID

  • Comfortable clothing you can move in (ideal for dry needling, chiropractic, and movement assessment)

  • Any recent X-ray reports, MRI reports, or labs if you have them

 

Wearing flexible or athletic-style clothing helps your first visit go smoothly. You can also layer. 

3. Arrival Info + Parking

When you arrive:

  • Park in the lot at the front of the building

  • Walk toward the computer store on the front side

  • You’ll see Suite 104 on the right-hand side

 

If you have trouble finding the office, feel free to text or call. Have a seat in the waiting room and I'll be out to get you shortly!

4. What to Expect on Your First Visit

 

Your first appointment is the Vital Flow Intake + Reset, which typically lasts 45–60 minutes.
During this visit, we will:

- Discuss your symptoms, health history, and goals
- Assess posture, movement patterns, tension, and nervous system function
- Perform your initial Vital Flow Reset (chiropractic + advanced dry needling)
- Create a clear plan based on what your body needs next

 

Most new patients choose this visit if they are experiencing:

  • Chronic tension

  • Stress-related body symptoms

  • Pain that hasn’t improved with other care

  • Feeling “off,” overwhelmed, or under-recovered

  • Tightness from activity, exercise, or desk work

Fields of Health Chiropractic is an out-of-network provider. You may request a superbill for potential reimbursement (not available for Medicare or Medicaid).
